Submitting claims by mail:
To file a claim with Cigna Healthcare, begin by filling out the appropriate form. Then use the checklist below to ensure you have everything you need.
Medical Claim Form:

Mail the claim to the address on the back of your Cigna ID card
Submitting claims online:
You can submit claims for reimbursement online by going to the Claims tab and Forms Center.
Here are the steps for submitting and completing claim submissions:
Hover over the Claims tab at the top
Click Forms Center from the drop-down selections
Under the heading Claims Forms, select Submit a Claim Online
Select the Coverage Type related to the claim you are submitting
Review the information needed to submit a claim online and select Start
Identify whom the claim is for
Identify if the claim is due to an auto accident or work-related illness/injury
Identify if the patient is covered by Medicare
Identify if the patient has other insurance
Upload claim documents (file size limit: 10 MB; file types accepted; JPEG, PNG, TIF, PDF, or photo) by clicking Choose a File
You should see the file name listed next to a button that says
Upload file; click this or the Next button will be greyed out
The next screen will give the option to Upload another file or Next
Review claim details and payment authorization: if the provider is in-network, payment will always be made to the provider
Check the certification box to Submit; you will then be directed to the confirmation page
Note: Please allow 30 calendar days for the claim to fully process; this will apply to both in- and out-of-network claims/providers.
What do you need?
Cigna requires an itemized bill to process the claim correctly.
They cannot accept receipts, balance due statements or cancelled checks in place of the itemized bill.
· Primary customer name
· Date of service (mm/dd/yyyy)
· Patient name
· Type of service/Procedure code
· Charge for the service
· Health care provider name/credentials
· Health care provider address
· Health care provider Tax ID number
· Diagnosis code (ICD format)
What to expect when filing an out-of-network claim:
Out-of-network claims will be priced based on the lesser of the provider’s normal charge for a similar service or supply. Or a percentile of charges made by providers of such service or supply in the geographic area where they are received (as compiled in a database selected by Cigna Healthcare).
If sufficient charge data is unavailable in the database for that geographic area to determine the Maximum Reimbursable Charge (MRC) , then state, regional or national charge data may be used.
If sufficient charge data is unavailable in the database for that geographic area to determine the (MRC), then data in the database for similar services may be used.
The provider may bill you for the difference between the provider’s normal charge and the (MRC), in addition to applicable deductibles and coinsurance.
