Your benefits begin on the first of the month following your date of hire, with the exception of disability coverage which begins on the first of the month following 6 months of service.
When is My Coverage Effective?
Open Enrollment will be conducted October 30 – November 10, 2017. This year we are pleased to announce we will have Benefits Educators with DirectPath, LLC available either onsite or through the DirectPath Benefits Service Center during open enrollment to speak one-on-one with each eligible employee. This will be your only opportunity to enroll in or make changes to your benefits program for the 2018 plan year. Beginning October 23rd, you may schedule your appointment online or by telephone. You may schedule your enrollment appointment on this site, or if you need assistance, please contact the Benefits Service Center at 844-307-9174, Monday – Friday (9:00 AM – 6:00 PM EST). If Educators are available on-site at your location, you will meet with the Benefits Educator face-to-face during your designated appointment time. If you schedule a phone appointment, a Benefits Educator will call you within 15 minutes of your selected appointment time. These are trained, non-commissioned Benefits Educators who will answer questions about the benefits program and help you enroll in your benefits for 2018. They are trained to respect confidentiality and will not disclose any of your confidential information. Take Action! This is your only chance to enroll in or make changes to your benefits for the 2018 plan year. When and How Do I Enroll?
For most of these benefits, you may change your coverage during the year only when you experience a qualifying event, such as: • Marriage, divorce or legal separation • Birth, adoption or legal guardianship of a child • Loss of coverage or eligibility of other coverage • Death of a covered dependent • Change in your/your spouse’s employment status • Change in dependent’s eligibility for benefits
Changing Coverage During the Year
All regular full- and part-time employees scheduled to work at least 24 hours per week are eligible for benefits. Regular employees working less than 24 hours per week may be eligible for other programs, such as the 401k Plan. If you are eligible for benefits, you may choose to cover the following dependents on your medical, dental, supplemental vision and life insurance. If you are adding dependents, proof of dependent eligibility must be submitted to Corporate Benefits at benefits.us@crl.com within 30 days of your eligibility date. Eligible Dependents Include: • Legal spouse • Natural, step, legally-adopted foster children and any other child who lives with you and for whom you are the legal guardian through the end of the month in which they turn age 26 • Unmarried disabled dependent children of any age, provided that proof of disability has been established prior to age 26 If you have specific questions about your work hours and eligibility, please contact Corporate Benefits at benefits.us@crl.com.
Who is Eligible?
Network: A group of hospitals or doctors that has contracted with the insurance company to provide services at a discounted rate. Because they offer their services for less, the plan provides higher coverage when you see these providers. Preventive Care: Includes things like wellness exams, routine tests and screenings, and certain prescriptions. Copay: A fixed dollar amount you pay for services such as doctor visits, prescriptions or emergency room visits. You generally pay this at the time of the service, so there are no bills that come later. Deductible: The amount you pay each year before your plan begins to pay for benefits. Coinsurance: A percentage of the cost that you pay for certain covered services. Here the actual cost of the service does matter; 10% of $200 is going to be different from 10% of $2,000. You are generally billed for this after the service has been received. Out-of-Pocket Maximum: The maximum amount of money you will pay out of your pocket for covered services during a coverage period (including the deductible, coinsurance and copays).
Helpful Terms & Definitions
Eligible dependents for Medical, Dental and Vision coverage include: • Your legal spouse. Keep reading for specific restrictions on eligibility requirements for employed spouses. • Children up to age 26. Includes birth children, stepchildren, legally adopted children, children placed for adoption, foster children, and children for whom legal guardianship has been awarded to you or your spouse. • Dependent children, regardless of age, provided he or she is incapable of self-support due to a mental or physical disability, is fully dependent on you for support as indicated on your federal tax return, and is approved by your Medical Plan to continue coverage past age 26. If both you and your spouse are employed by Ensemble or any affiliate, only one of you may cover your eligible child(ren). Verification of dependent eligibility will be required after enrollment.
Who Can Be Covered Under Your Plan
If your spouse is eligible under another employer’s medical plan at a cost of $180 each month or less for individual coverage for the lowest-cost plan option, your spouse must enroll in primary coverage through his/her employer to be eligible for secondary coverage under the Ensemble Medical Plan. Standard Coordination of Benefits rules would apply. You will be asked to verify that your spouse does not have other medical coverage.

If you receive a bill from your doctor or if you are paying for a prescription, you can pay from your HSA using your HSA debit card or checks provided by your bank.
How do I pay with an HSA?
Yes, as long as the service is a qualified expense. You can take money out of your HSA to pay yourself back with no penalty.

The effective date of your benefits elections is July 1, 2017.
All employees scheduled to work at least 30 hours per week are eligible to participate in the Waste Industries benefits program. Eligible employees may also enroll their legal spouse, if not eligible for their employer plan, and dependent children (married or unmarried) who are under age 26. A dependent child may be the natural child, stepchild, legally adopted child, child placed for adoption, or other child for whom the employee has permanent legal custody. Dependent eligibility for a spouse or child over the age of 19 is determined through a certification verifying there is no access to other group insurance. If you are covering a spouse or child dependent over the age of 19, you will be required to complete a new verification form for the 2017-18 plan year.
You can change your coverage during the year when you experience a qualified change in status, such as marriage, divorce, birth, adoption, placement for adoption, or loss of coverage. The change must be reported to your Human Resources Department within 30 days of the event. The change must be consistent with the event. For example, if your dependent child no longer meets eligibility requirements, you can drop coverage only for that dependent.

For members up to age 19 – one set of prescription lenses and/or frames or contact lenses per member every two calendar years (15% coinsurance will apply). PPO plans provide you the freedom to access care from any doctor, hospital or other provider. However, when you use a BCBS Blue Care Elect PPO network provider, your out-of-pocket costs will generally be lower. This is because in-network doctors have agreed to offer their services for lower, pre-negotiated rates. Preventive services and related tests are covered at 100%. You do not have to choose a primary care physician, and you do not need a referral to see a specialist.

ID Cards: You will receive one card for medical and prescription drug coverage. Each family member will receive his or her own ID card in their own name. ID cards are issued with the 3-letter prefix (LBS) and a unique 9-digit subscriber number. The card(s) will be mailed to your home address. Please check to be sure that all of the information on the card is correct. Remember to present this card during your first office visit or pharmacy purchase so your claims will process correctly.

How It Works
Participants may save the most money and receive the highest level of coverage when they visit a Delta Dental PPO dentist. Our PPO dentists have agreed to accept lower fees as full payment for covered services. Participants may still save money if they visit a Delta Dental Premier dentist who agrees to accept Delta Dental’s fee determination as full payment for covered services.

To help you manage dental costs, Charles River offers a comprehensive dental plan through Delta Dental of Massachusetts.
Our plan is called the Delta Dental PPO Plus Premier Plan. Delta Dental offers two nationwide networks – the Premier and the PPO – and you can use providers from both. The Premier network provides access to more than 80% of dentists nationwide. These dentists have agreed to offer their services for a lower, contracted rate. The PPO network is smaller (about 50% of dentists nationwide), but these providers have agreed to offer even bigger discounts. Your out-of-pocket costs will be lower when you receive care from a Delta Dental PPO network provider.

You will receive one ID card in your name if you elect single coverage. If you elect to cover any dependents, you will receive two ID cards in your name, and they can be used by everyone covered on your plan. Rollover Max: Rollover Max allows you to roll over a portion of your unused benefit dollars from one year to the next. Each covered family member can roll over up to $600 as long as they have one cleaning or oral exam in the plan year and their total plan year claims don’t exceed $800. The accumulated rollover total is capped at $1,500.

Vision Rates
The IRS has strict plan guidelines to determine who is eligible to own and contribute to an HSA. First, you must be enrolled in a high deductible health plan. The PPO plus HSA is the only HSA-compliant health plan, so you must enroll in the PPO plus HSA if you want to open an HSA. In addition, you cannot be covered by another health plan (including Medicare), you cannot have access to dollars in a Health Care Flexible Spending Account (including a spouse’s FSA), and you may not be claimed as a dependent on someone else’s tax return.
Who is eligible to enroll in an HSA?
You build your account balance through employee and employer contributions, subject to annual limits set by the federal government. For 2018, the contribution limit (employee and employer combined) is $3,450 for individual coverage and $6,900 for all other coverage levels. How much can I contribute to my HSA account?
Contributions are made on a pre-tax basis through payroll deductions. You do not have to make employee contributions to be eligible for the employer contribution. Simply elect $0 for the HSA when enrolling.
How do I contribute to my HSA account?
The Health Savings Account is managed through HealthEquity. To use the funds in your HSA for qualified medical expenses, you may: 1. Use the “Pay My Provider” feature on the HealthEquity website to send an online payment to your medical provider. 2. Use the HealthEquity-issued debit card to pay for eligible expenses at the point of purchase. 3. Pay for your expenses with other funds and reimburse yourself with the HSA dollars. Qualified health care expenses may be incurred by you, your spouse or any of your eligible dependents and include: • Deductibles • Copays and coinsurance • Over-the-counter drugs (with a prescription) • Glasses and contact lenses • Laser eye surgery • Dental and orthodontic expenses (excluding cosmetic procedures)
How do I use my HSA funds?
All of the money in your HSA remains yours even if you leave the company, move out of the PPO plus HSA or retire. Unused money is never forfeited. So, if you don’t use all the money for today’s medical expenses, you can save it for your future expenses. Once your HSA balance reaches $2,000, you can choose to invest in mutual funds to help maximize your savings.
What happens to my HSA if I leave the company?
You can contribute to an HSA if you: • Are 18 or older; • Have coverage under an HSA-qualified medical plan; • Have no other first-dollar medical coverage, which means no other comprehensive major medical insurance policy, a health flexible spending account (Health FSA), or a health reimbursement arrangement (HRA) – unless this coverage is limited to benefits not covered by your health plan. Other types of insurance like specific injury insurance or accident, disability, dental care, vision care, or long-term care insurance are permitted; • Are not enrolled in Medicare; and • Cannot be claimed as a dependent on someone else’s tax return.

A Health Savings Account (HSA) is a tax-free way to save for qualified health care expenses. Contributions are made pre-tax, funds grow tax-deferred and withdrawals for qualified medical expenses are tax-free. The Charles River HSA is administered by HealthEquity (healthequity.com).
Employer Contributions - Charles River will contribute to your HSA based on your health plan coverage level: $500 for Employee Only and $1,000 for all other coverage levels. The employer matching contribution will be prorated and determined by the month you become eligible for benefits.

This program allows you to pay for child and elder care expenses when you and your spouse work or attend school full-time. Eligible expenses include: • Licensed nursery schools and day care centers for preschool children • Day camps, before- and after-school care or in-home day care for children under age 13 • Day care centers for other qualifying dependents, such as elder care centers Dependent Care FSAs carry a “use it or lose it” provision. If you do not use all the funds for expenses incurred during the plan year, you will forfeit these funds.
Dependent Care FSA
This program allows you to pay for mass transit and parking expenses incurred during your commute to and from work. You must enroll in the commuter benefit online at wageworks.com by the 10th of the month in advance of the month you wish to participate unless you schedule the transaction as an automatic, recurring monthly purchase.
Commuter Benefit
You can submit claims and attach documentation online by logging into your account at www.ebcflex. com. If you have an Android smartphone or Apple iPhone, you can download “My Mobile Account Assistant.” It lets you view your account balance, submit claims and attach documentation. Download it from GooglePlay or the Apple App Store. Claims for daycare expenses can only be submitted to us up to 8 weeks in advance. However, you are not reimbursed until after the last day of service listed on your claim. For example, if you date-span your request to encompass 8 weeks, your payment will not be issued until the last day of the eighth week. To be reimbursed as often as possible, one solution would be to file each week as a separate line item.

A. Charges for daycare services outside your home for a “qualifying child” who is under the age of 13 and who depends on you (and your spouse, if you are married) for at least half of his/her support, does not have his/her own dependents, and is not a “qualifying child” of any other taxpayer during the year B. Charges for care outside of your home for your spouse, dependent adult or child who is mentally or physically incapable of caring for himself or herself and has the same principal place of abode; the spouse or dependent must spend at least 8 hours of each day in your house C. You may be reimbursed for expenses to provide care to the individual(s) described above in your home if the services are, at least in part, so you (and your spouse, if you are married) may work; the expenses include wages paid to the service provider, but not expenses such as food or clothing In the case of divorce, legal separation or parents living apart, special rules apply. In general, the custodial parent is the one with whom the child has resided with for the longest period of time or with whom the child has spent the most evenings during the year. The following partial list of services are examples only and must comply with the rules listed above in order to be eligible for reimbursement.

There are other Qualifying Events that allow you to make changes to the Dependent Care FSA.

A fundamental element of any benefits package is life insurance which is designed to protect you and your loved ones from financial hardships related to an untimely death. NMHS provides you with $250,000 of Basic Life Insurance. This benefit is provided to you at no cost. When you are age 70 or older, your Basic Life Insurance benefit will reduce to 50% of the benefit at age 70. Spouses and dependents will receive $10,000 of coverage.
Basic Life for Physicians
A fundamental element of any benefits package is life insurance which is designed to protect you and your loved ones from financial hardships related to an untimely death. NMHS provides you with 1 times your annual compensation rounded to the next higher $1,000 up to $50,000 of Basic Life Insurance. This benefit is provided to you at no cost. When you are age 70 or older, your Basic Life Insurance benefit will reduce to 50% of the benefit at age 70. Spouses and dependents will receive $10,000 of coverage.

Charles River offers you the opportunity to purchase additional life insurance through Cigna over and above what is provided to you by Charles River. Regular employees
In the course of your working life, you are more likely to become ill or injured and need disability coverage than you are to die and need life insurance.
Disability Coverage
If you are medically unable to work for more than seven (7) days, you may be eligible for disability income protection for up to 26 weeks. The initial 7-day elimination period may be paid through any available time off. The STD income replacement percentage is based on your years of service. You are automatically enrolled in STD on the first of the month following 6 months of employment.
Short-Term Disability (STD)
*Any Injury or Sickness for which you incurred expenses, received medical treatment, care or services including diagnostic measures, took prescribed drugs or medicines, or for which a reasonable person would have consulted a Physician within 12 months before your most recent effective date of insurance. The Pre-existing Condition Limitation will apply to any added benefits or increases in benefits. This limitation will not apply to a period of Disability that begins after you are covered for at least 24 months after your most recent effective date of insurance or the effective date of any added or increased benefits.
Charles River now offers employer-paid core LTD paired with a voluntary buy-up option. The core plan replaces 50% of your monthly wages up to $12,500 per month. This payment may be reduced by certain state disability earnings. The duration of your benefit is based on your age when the disability occurs. LTD benefits are not taxable income. MetLife's Group Critical Illness Insurance can help protect your finances from the expense of a serious health problem, such as a stroke or heart attack. This coverage can complement existing medical coverage and help fill financial gaps caused by out-of-pocket expenses such as mortgage payments, college tuition, hiring household help, or treatment not covered by your medical plan. Cancer coverage is also included.

The Group Legal Plan, also known as MetLaw, is a voluntary, after-tax benefit that provides personal legal services to you and your eligible dependents. Employee Stock Purchase Plan
Purchase shares of Charles River common stock through payroll deduction without paying broker fees and administrative costs. The minimum bi-weekly, after-tax deduction is $20. Stock is purchased on the last business day of the calendar quarter, and shares are held in an account in your name at UBS Financial Services. Contact your HR Manager or visit the Benefits Community on iConnect for the Stock Purchase Plan Enrollment and W-9 forms, which must be completed and returned to Corporate Benefits. Questions should be directed to UBS at 800-820-9296. After your initial stock purchase, you will receive a welcome letter from UBS with instructions on how to access your account. 401(k)
Charles River offers a 401(k) Plan to help you save for retirement. The plan is administered by Fidelity, and you may contribute from 1% to 60% of your pre-tax, bi-weekly pay, up to $18,500 in 2018. Pre-tax contributions reduce your current taxable earnings, but you must pay taxes on your contributions and any investment earnings when you take a distribution. The plan provides a variety of investment options to help you meet your retirement goals. It permits rollovers and includes provisions for loans and hardship withdrawals. Employer Matching Contribution: The Company will match 100% of the first 3% that you contribute and 50% of the next 2% that you contribute. You must contribute in order to receive matching contributions. The matching contribution will be computed based upon your eligible compensation contributed to the plan each pay period. Therefore, to maximize the Company matching contribution, you must spread your contributions through the entire calendar year. You are always 100% vested in your employee contributions and the employer contributions. Enrolling in the Plan: Shortly after your date of hire, Fidelity will mail you a Retirement Plan Enrollment Kit. If you do not take action within 30 days of your hire date, you'll be automatically enrolled in the plan with a 3% employee contribution and a 3% employer contribution.
