Best for those who want the most freedom to choose their dentist.
- You pick your dentist from the largest provider networks: state and national.
- Get specialist care without a referral.
- Deductible waived for preventive care and children under age 15.
- Posterior composite filling coverage.
- Enhanced crown coverage.
- Coverage for out-of-network care.
DeltaCare®
Managed Care Plan
Best for those who want no annual maximum limit and fixed out of pocket expenses.
- Requires primary care dentist (PCD) from the DeltaCare network.
- DeltaCare PCD must refer you for any specialist care.
- No deductibles.
- Unlimited coverage for fillings, root canals, crowns & other services.
- No coverage for out-of-network care.
- This plan has a copay, you pay a fixed dollar amount for each treatment you receive.

Nonsurgical TMJ
30% up to $1,000 per year or $5,000 lifetime annual maximum. Patient pays any cost exceeding lifetime annual maximum.
Nonsurgical TMJ
30% of costs until the plan has paid $1,000 (for PPO, out of state, and non-PPO providers); then any amount over $500 in the member's lifetime (maximum lifetime benefit).
Orthodontics
50% until you reach lifetime annual maximum of $1,750; patient is responsible for any amount exceeding the liftime annual maximum.

Want to change your primary care dentist?
- Log into your MySmile account
- Click “Change Assigned PCD” link
- Select member or dependent name, enter zip code and click “Search”
- Highlight the dentist you want and click “Select”
Click “Confirm”
Changes made on or before the 20th of the month will be effective on the first day of the next month.
Changes made after the 20th of any month are effective the first day of the month after next.
The DeltaCare plan requires patients to select a primary care dentists within the DeltaCare network in order to receive any benefit from the plan.
You are encouraged to select a primary care dentist upon enrollment.
You can change your primary care dentist at anytime online. Learn more.
For some specialty care, a network primary care dentist must refer the member to a network specialist.
To get the most savings and avoid extra paperwork, choose a dentist in the Delta Dental PPO network. You'll also be assured your dentist meets our quality standards.
If your dentist is not part of the Delta Dental network:
- You'll be responsible for any difference between your provider’s charge and Delta Dental’s allowable fee, in addition to your coinsurance and deductible.
- Out-of-network dentists may require full payment upfront.
- Out-of-network dentists may require you to file your own claims with Delta Dental.
