BJ’s provides benefit plans that give you and your family choice. BJ’s offers rewards for those who promote their own wellness by getting their preventive screenings and completing the Cigna Health Assessment. Federal Health Care Reform guidelines mandate that employers offer insurance to Team Members who work 30 standard hours or more per week. The Value Prescription Drug List promotes the use of generic and lower-cost brand medications to help you stay healthy and save money. Generic medications offer the same strength and active ingredients as the brand name but often cost much less – in some cases, they cost 80%–85% less. Specifically, two categories of medications will be excluded as they can be found over the counter. These categories are heartburn/stomach and allergies. Here are a few examples of medications that can be found over the coun ter and will no longer be on Carrier’s formulary.
Value Prescription Drug List (PDL)
Generic prescriptions will be required unless otherwise specified by your doctor. Generic medications provide you with the same quality, strength, purity and stability as the brand name but often cost much less. If your doctor writes your prescription as Dispense as Written (DAW) because you cannot take a generic, you will receive a brand prescription at the brand copay. No other charges will apply. If you REQUEST a brand prescription at the pharmacy and there is a generic available, you will be charged a penalty. The penalty is the brand copay (found on pages 4 and 5) plus the difference between the cost of the brand prescription and the generic prescription.

You must re-elect your FSA accounts every year.
BJ’s offers Flexible Spending Accounts (FSA) for health care and dependent care through WageWorks. The FSA plan allows you to set aside pre-tax dollars to cover qualified expenses you would normally pay out of your pocket with post-tax dollars. Once an annual election is made, you cannot opt out or decrease your elected amount. In the case of a qualifying life event, such as the birth of a child, you may increase your annual contribution.

BJ’s provides Short-Term Disability (STD) to eligible Team Members to cover a disability up to 26 weeks. STD Provides income replacement if you are disabled more than XXXX (X) calendar days. Team Members may elect to use accrued sick time, vacation time and personal time during the first XXXX (X) calendar days of disability, known as the “elimination period.” Sick time will be charged based on your standard hours during the seven day period. Please refer to your enrollment guide for more detailed information on this plan or see your HR Department.
BJ’s offers Short-Term Disability (STD)
This benefit helps you in the event you become unable to work due to a non-work related illness or injury.
* Salary does not include overtime, bonus or commissions, but do include car allowance, if any, up to a maximum of $XXXX.

LTD insurance can supplement your income in the event you remain disabled longer than XX weeks. The LTD benefit will be offset by any other disability income you may be eligible for (Social Security, Workers’ Compensation, State-Provided Disability, etc.) and will be based on the following: • XX% of your basic monthly earnings • $XX maximum monthly benefit (tax-free) Since the cost for this benefit is paid entirely by you, the premium is withheld on an after-tax basis. Any benefit payments are paid tax-free. Benefits are paid by Liberty Mutual on a monthly basis.

Age Reductions
During Annual Benefits Enrollment, you may choose to increase this coverage up to the guaranteed issue amount (lesser of three times your annual earnings or $XXXX) without showing Evidence of Insurability (EOI).
You may elect to purchase Supplemental Life Insurance for your spouse and/or dependents.
You may purchase AD&D coverage in increments of $XXXX up to a maximum of 10 times your basic annual earnings, to a maximum of $XXXX for yourself and/or your family. AD&D pays an additional amount of Life Insurance if an accident occurs resulting in dismemberment or death.
Age Reductions will apply to the Team Member’s Basic Life and Supplemental Life amounts. There are no age reductions on Dependent Life or AD&D. Reductions start at age 65 and continue according to the schedule below: • At age 65 the benefit amount reduces to 65% of the original amount • At age 70 the benefit amount reduces to 50% of the original amount • At age 75 the benefit amount reduces to 35% of the original amount
You may also purchase Supplemental Life Insurance coverage in amounts up to five times your basic annual earnings to a maximum of $XXXX (cannot exceed $XXXX combined with Basic Life Insurance coverage).

• Coverage is available to eligible Team Members age 17 to 69 (to age 64 in CA and NY) who are actively at work.* • You can choose a monthly benefit between $400 and $5,000 for covered disabilities due to injury or illness. • Coverage of up to 50% of your gross monthly salary may be offered (coverage in NJ, NY and RI is limited to 40%). • The affordable premium is based on your age when you buy the insurance and will not increase as you get older.** • Your policy is guaranteed renewable, until age 72, as long as you pay the premiums on time. • Your plan includes a Waiver of Premium, included at no extra charge, for covered injuries and illnesses. This means you don’t have to pay your premiums after 90 days of total disability or the elimination period (whichever is longer). They’ll be waived as long as the disability continues, up to the maximum benefit period. • This plan includes convenient payroll deduction, so you don’t have to remember to write a check for your premiums. • You own the policy so you can keep this coverage if you leave the company or retire. Unum will bill you directly. • Coverage becomes effective on the first day of the month in which payroll deductions begin.

Carrier’s Individual Short Term Disability Insurance protects a portion of your income if you are unable to work due to a covered injury or illness after 26 weeks.
This coverage can pay a monthly benefit to provide some income during a time of need. Common reasons people use this coverage include injuries, a covered pregnancy and digestive problems — such as gall bladder surgery.

Could your bank account survive a serious illness?
Carrier’s Critical Illness Insurance can help protect your finances from the expense of a serious health problem, such as a stroke or heart attack. Cancer coverage is also available. You choose a lump-sum benefit up to $XX that’s paid directly to you at the first diagnosis of a covered condition. You can use the benefit any way you choose and more than once. If you receive a full benefit payout for a covered illness, your coverage can be continued for the remaining covered conditions. The diagnosis of a new covered illness must occur at least XX days after the most recent diagnosis. Each condition is payable once per lifetime.
