When Is My Coverage Effective?
The effective date of your benefit elections is January 1, 2017.
Changing Coverage During The Year
For most of these benefits, you may change your coverage during the year only when you experience a qualifying event, such as marriage, divorce, birth, death, adoption, placement for adoption or loss of coverage. You have 31 days to report the change to your HR Department. The change must also be consistent with the event. For example, if your dependent child no longer meets eligibility requirements, you can drop coverage only for that dependent. You can also submit your Life Event request online at www.dibenefits.bswift.com.
How to Enroll?
All full-time employees working 30 or more hours per week are eligible to participate in the Distribution International benefits program. Eligible employees may also enroll their legal spouse and/or dependent children. A dependent child may be the natural child, stepchild, legally adopted child, foster child or other child under the age of 26 for whom the employee has permanent legal custody. If you have specific questions about your work hours and eligibility, please contact your HR department.
This year we are pleased to announce we will have Benefits Educators with DirectPath, LLC available either onsite or through the DirectPath Benefits Service Center during open enrollment to speak one-on-one with each eligible employee. This will be your only opportunity to enroll in the benefits program for the 2017 plan year. Beginning Monday, November 21st, you may schedule your appointment on this site or by telephone. If you need assistance or do not have internet access, please contact the DirectPath Benefits Service Center at 855-687-2164 (Monday – Friday, 8:00 AM – 5:00 PM CST). If you schedule an onsite appointment, you will meet with the Benefits Educator face-to-face during your designated appointment time. If you schedule a phone appointment, a Benefits Educator will call you within 15 minutes of your selected appointment time.
Open Enrollment is your opportunity to review your benefits coverage and make choices for the upcoming plan year. All benefits-eligible employees are required to meet with a Benefits Educator to continue, waive or change benefits for 2017. These are trained, non-commissioned Benefits Educators who will answer questions about the benefits program and help you enroll in your benefits for 2017. They are trained to respect confidentiality and will not disclose any of your confidential information. Please read the information in this site carefully and consider each product offering. The benefits program will allow you to protect your income, health and family in times of need.

Flexible Spending Account
To access your funds, you will be issued a debit card from UMR. UMR may at times require you to substantiate your purchases, so keep your receipts!
A flexible spending account (FSA) is an account you establish to pay for health and dependent care expenses with pre-tax dollars.
• Choose a specific amount of money to contribute each pay period, pre-tax, to one or both accounts during the year. • The amount is automatically deducted from your pay at the same level each pay period. • As you incur eligible expenses, you may use your flexible spending debit card to pay at the point of service or submit the appropriate paperwork to be reimbursed by the plan. Important Rules to Keep In Mind: • The IRS has a strict “use it or lose it” rule. You will forfeit any funds left over after the end of the 2 1/2 month grace period. You have until March 15th to incur claims and use the monies from the previous year. • Once you enroll in the FSA, you cannot change your contribution amount during the year unless you experience a qualifying event. • You cannot transfer funds from one FSA to another.

Important Rules to Keep In Mind
You may contribute up to $2,600 per plan year in the Health Care FSA, allowing for certain IRS-approved health expenses not covered by an insurance plan. These expenses include copays, deductibles, coinsurance, hearing aids and contact lenses. For a complete listing of eligible expenses, please see Human Resources or review IRS Publication 502 available online at www.irs.gov. Please Note: Health Care FSA funds can be used on yourself or eligible dependents.

FSA Example
You may contribute up to $5,000 pre-tax to this account. The dependent care account may be used to pay for dependent care expenses for dependents up to age 13 or elderly dependents. Qualifying expenses include day care fees, before-school and after-school care and summer day camp. If you are married, your spouse must either be employed or a full-time student in order to use a dependent care flexible spending account. Under IRS guidelines, you can only be reimbursed for dependent care that has already taken place. Also, you can only be reimbursed for the amount you have already contributed to your dependent care FSA. For a complete listing of eligible expenses, please see Human Resources or review IRS Publication 503 available online at www.irs.gov.

To select your benefit amount and calculate your premiums, do the following: 1) Locate the benefit amount you want to select from the top row of the employee premium table. Your benefit amount must be in an increment of $10,000 (ex. $10,000, $20,000, or $50,000). Refer to the Coverage Guidelines section for minimums and maximums, if needed. 2) Find your age bracket in the far left column 3) Your premium amount is found in the box where the row (your age) and the column (benefit amount) intersect. 4) Enter the benefit and premium amounts into their perspective areas in the Voluntary Life and AD&D section of your enrollment form. If the benefit amount you want to select is greater than $100,000. Select the benefit amount from the top row that when multiplied by another number results in the benefit amount you want to select. For example, if you want $150,000 in coverage, you obtain your premium by multiplying the rate for $50,000 times 3.

A fundamental element of any benefits package is life insurance which is designed to protect you and your loved ones from financial hardships related to an untimely death. NMHS provides you with $250,000 of Basic Life Insurance. This benefit is provided to you at no cost. When you are age 70 or older, your Basic Life Insurance benefit will reduce to 50% of the benefit at age 70. Spouses and dependents will receive $10,000 of coverage.
Basic Life for Physicians
A fundamental element of any benefits package is life insurance which is designed to protect you and your loved ones from financial hardships related to an untimely death. NMHS provides you with 1 times your annual compensation rounded to the next higher $1,000 up to $50,000 of Basic Life Insurance. This benefit is provided to you at no cost. When you are age 70 or older, your Basic Life Insurance benefit will reduce to 50% of the benefit at age 70. Spouses and dependents will receive $10,000 of coverage.
Basic Life for All Other Employees

You may also purchase additional voluntary life insurance through Mutual of Omaha to meet your personal family’s needs. Protecting your family’s future is of the utmost importance, and this plan provides an excellent opportunity to purchase affordable coverage at a low group rate. Voluntary Life and AD&D Insurance

Short-Term Disability Insurance
Distribution International also provides long-term disability coverage at no cost to you after you have satisfied your waiting period. If you have an illness or injury that lasts longer than 90 days, this plan replaces 60% of your base monthly pay, up to a monthly maximum of $5,000. It can help you pay your bills and protect your finances at a time when you have extra medical costs but don’t get a paycheck. Please review the plan overview below.

WELLNESS BENEFIT! Receive $50 per calendar year per insured individual if a covered health screening test is performed, including blood tests, stress tests, colonoscopies, mammogram, or X-rays. A full list of covered tests will be provided in your certificate.
Covered conditions: • Heart attack • Blindness • Major organ failure • End-stage renal (kidney) failure • Occupational HIV • Coronary artery bypass surgery (pays 25% of lump- sum benefit) • Benign brain tumor Covered conditions with time limitations: • Stroke (evidence of persistent neurological deficits confirmed at least 30 days after the event) • Coma (resulting from severe injury lasting 14 consecutive days or more) • Permanent paralysis (complete and permanent loss of the use of two or more limbs for a continuous 90 days as a result of a covered accident) You may choose to select this benefit for an additional premium: • Cancer • Carcinoma in situ* (pays 25% of the lump-sum benefit)

Advantages of the Plan
Unum’s Group Critical Illness Insurance can help protect your finances from the expense of a serious health problem, such as a stroke or heart attack. Cancer coverage is also available. You choose a lump-sum benefit up to $50,000 that’s paid directly to you at the first diagnosis of a covered condition. You can use the benefit any way you choose.

Would a trip to the hospital leave your finances in serious condition?
• $1,000 each covered hospital admission once per year • $100 each day of your covered hospital stay, up to 15 days once per year • $200 each day you spend in intensive care, up to 15 days once per year
You may receive a benefit for the following:
Unum’s Group Hospital Indemnity Insurance can complement your health insurance to help you pay for the costs associated with a hospital stay. It can also provide funds for the out-of-pocket expenses your medical plan may not cover, such as coinsurance, copays and deductibles. How does it work? This insurance pays a benefit when you are admitted* to the hospital for a covered accident or sickness. *Covered individuals must be hospitalized for at least 20 hours.
• Coverage is available to all eligible employees who are actively at work.* • You can buy coverage for your spouse and dependent children. • No health questions to answer. If you apply, you automatically receive the base plan. • This plan includes convenient payroll deduction, so you don’t have to remember to write a check for your premiums. • Coverage is portable. You may take the coverage with you if you leave the company or retire without having to answer new health questions. Unum will bill you directly. • Coverage becomes effective on the first day of the month in which payroll deductions begin. • Benefits are paid for accidents that occur on and off the job. • A Catastrophic Benefit is included with this plan. This pays an additional sum if a covered individual has a serious injury — such as loss of sight, hearing or a limb. Benefit amounts reduce after age 65.
