If I elected coverage last year, will my coverage automatically roll over? Yes, you will also have an opportunity to increase the coverage amount if you wish. Will there be extensive medical underwriting involved if I elect coverage? No, you can elect coverage up to the guaranteed issue with no medical underwriting. Can I add my spouse if they are not currently covered? Yes, you may add your spouse if they are not currently covered up to the last day of the 2018 Benefits Open Enrollment. After that, you must wait for the next open enrollment period. Can I change my address? Yes, you may change your address by calling the appropriate carrier. Can I update my beneficiary? Yes, you may update your beneficiary by calling the appropriate carrier. Can I add my child(ren) if they are not currently covered? Yes, you may add your children if they are not currently covered up to the last day of the 2018 Benefits Open Enrollment. After that, you must wait for the next open enrollment period. What if I did not receive my policy? Accident and Critical Illness, Unum is a green company. They mail a letter with instructions on how to download a copy of the policy. When and How to Enroll?
To enroll, login to FMC4ME.com from October 16 - November 10 and select the Benefits Open Enrollment link. If you are adding new dependents, you must verify their eligibility by November 10, 2017. Elect or waive your voluntary benefits – Critical Illness, Accident, Hospital Indemnity, and Universal Life – to complete your enrollment. You may do this by clicking the Voluntary Benefits link within PeopleSoft, or by scheduling an appointment with a Benefits Educator. Once you schedule your telephonic appointment, a Benefits Educator will call you within 15 minutes of your designated appointment time. Be sure to have your dependent and beneficiary information (Social Security number and date of birth) with you at the time of your appointment. During your appointment, the Benefits Educator will explain the voluntary benefits available to you and enroll you in the benefits of your choice.
When is Coverage Effective?
Universal Life Insurance is effective upon the date of application, pending underwriting approval. Accident, Critical Illness, and Hospital Indemnity Insurance are effective January 1, 2018.
Who Is Eligible for Voluntary Benefits?
All benefits-eligible employees scheduled to work at least 30 hours per week are eligible to participate in the voluntary benefits. Eligible employees may also enroll their legal spouse, domestic partner, and dependent children and grandchildren. Appropriate proof of domestic partnership must be submitted to FMCNA in order for a domestic partner to be enrolled in coverage. A dependent child may be the natural child, stepchild, legally adopted child, child placed for adoption, or other child for whom the employee has permanent legal custody. Dependent children are covered up to age 26 regardless of marital or student status. Coverage for dependent children ends at the end of the month in which they attain age 26. The exception to this is with Trustmark Universal Life Insurance, which covers dependent children up to age 23. Note: Employees covered under a Collective Bargaining Agreement may not be eligible for this program. During Open Enrollment, Benefits Educators will be available by phone October 16 - November 10 to answer your benefits-related questions and enroll you in your benefits for 2018. Sickness Hospital Confinement Benefit – This benefit pays the insured employee, spouse or child a daily benefit if he or she is in the hospital for a covered illness. The amount you receive is $100 per day for employees and spouse. Children’s coverage pays 75% of the employee amount. This benefit is available to family members who are covered by the base plan. There is an additional charge for this feature. Please note that the Sickness Hospital Confinement Benefit does have a pre-existing limitation of 12/12. This means that if you were treated for a medical condition 12 months prior to the effective date, it will not be covered unless you are treatment-free for 12 months after the effective date of coverage and are still actively at work for 12 months.
• Coverage is available to all eligible employees who are actively at work.* • You can buy coverage for your spouse and dependent children. • No health questions to answer. If you apply, you automatically receive the base plan. • This plan includes convenient payroll deduction, so you don’t have to remember to write a check for your premiums. • Coverage is portable. You may take the coverage with you if you leave the company or retire without having to answer new health questions. Unum will bill you directly. • Coverage becomes effective on the first day of the month in which payroll deductions begin. • Benefits are paid for accidents that occur on or off the job. • A Catastrophic Benefit is included with this plan. This pays an additional sum if a covered individual has a serious injury – such as loss of sight, hearing or a limb. Benefit amounts reduce after age 65.
Chiropractic services include $50 for each visit. You receive three visits per covered accident, per calendar year!

Unum’s group Critical Illness Insurance can help protect your finances from the expense of a serious health problem, such as a stroke or heart attack. Cancer coverage is also available. You choose a lump-sum benefit up to $50,000 that’s paid directly to you at the first diagnosis of a covered condition. You can use the benefit any way you choose.

Covered conditions with time limitations:

Advantages of the plan
• Wellness Benefit – this benefit pays $50 per calendar year per insured individual if a covered health screening test is performed, including blood tests, stress tests, colonoscopies, mammograms, chest X-rays. A full list of covered tests will be provided in your certificate. • Coverage is available to eligible employees who are actively at work.** You can buy coverage for your spouse ages 17–64 with purchase of employee coverage.1 Benefit amount is from $5,000 to $30,000 in $1,000 increments. • All eligible dependent children ages newborn until their 26th birthday, regardless of marital or student status, are automatically covered at 25% of the employee benefit amount at no additional cost. Eligible children are covered for the same conditions as the employee and the following specific childhood conditions: cerebral palsy, cleft lip or palate, cystic fibrosis, Down syndrome and spina bifida. Diagnosis must occur after the child’s coverage effective date. • You can use this coverage more than once. If you receive a full benefit payout for a covered illness, your coverage can be continued for the remaining covered conditions. The diagnosis of a new covered illness must occur at least 90 days after the most recent diagnosis. Each condition is payable once per lifetime. • You get affordable rates when you buy this coverage through your employer, and the premiums are conveniently deducted from your paycheck. • Coverage is portable. You may take the coverage with you if you leave the company or retire, without having to answer new health questions. Universal LifeEvents®: LifeEvents® is designed to match your needs throughout your lifetime. It pays a higher death benefit during working years when expenses are high and your family needs maximum protection. Then, at age 70, when financial needs are typically lower, the death benefit is reduced to one-third. Accelerated Death Benefit: The Accelerated Death Benefit provides 75% of the death benefit when life expectancy is 24 months or less. Optional Benefit - EZ Value Plan: The EZ Value Plan automatically increases coverage to keep pace with increasing needs – without additional underwriting. Coverage is available for employees and spouses through age 60 for an additional premium of $1 per week on each of the first five anniversaries.
