By submitting you and your employees’ information here, you hereby certify that 1) your company is a current member of the HBAA, and 2) that you understand all premiums for plans elected, whether by the member or an employee, must be remitted monthly by the member company no later than the 15th of the month, or the weekday immediately following.
If you previously completed the HBAA interest survey, you are not required to submit a new eligibility upload. Please note, items 1 and 2 from above apply. If you have a question regarding eligibility, please email HBAA@cobbsallen.com.
