Rafiki Coalition
Rafiki Coalition’s mission is to eliminate health inequities in San Francisco’s Black communities through education, advocacy, and by providing holistic health and wellness services in a culturally affirming environment.
Rafiki Coalition strives to achieve its focus by providing health and wellness services including, but not limited to, health education, movement classes, health screenings, advocacy, transitional housing and case management services for people living with HIV/AIDS, trauma resiliency and mental health circles, and other health-promoting activities.
Umoja Health
Umoja Health is a coalition of community and county public members, various organizations and healthcare providers working to increase access to community-based COVID-19 testing and vaccinations. The Bay Area Community Health Advisory Council (BACHAC) is an Umoja Health partner, and is dedicated to eliminating health disparities across generations and diverse communities through education and services. BACHAC has worked with corporations, mid- and small-sized companies, community-based organizations, nonprofit health organizations, governmental agencies, health educators, and the faith community, to create a village united around a common goal: increasing awareness of and reducing the debilitating effects of health concerns that disproportionately affect communities of color.
Community leaders and nonprofit organizations are removing barriers to help create a more socially, economically and racially just Bay Area, where people have the opportunity to shape their lives and communities. The Chan Zuckerberg Initiative (CZI) is committed to supporting this work, partnering with organizations that are activating community-centered solutions to address housing affordability, homelessness, education, and more; and that are building access for people to influence decisions that affect their communities.
