Insight’s technical services team provided superior knowledge complementing the council’s expertise to deliver and transition to thenew unified communications platform. Moving from physical telephones to soft-phones and headsets means staff can make and receive calls from any location. This provedessential for remote working, but it also means that fewer calls are missed, making it easier for citizens to access the council.
Why Insight?
As an accredited partner to over 2,100 top-tier manufacturers, Insight could offer a unified solution that would best meet the council’s needs without tying it into a specific vendor. North Ayrshire Council was assured that the solution would deliver both best fit and best value. Insight ensured that the new unified communications system would be compatible with the existing systems and meet the council’s future cloud-based telephony and cloud-based software-as-a-service roadmap. While North Ayrshire Council was able to update and future-proofits voice platform with SIP trunks in many locations, some sites inrural areas lacked ethernet connectivity capabilities required for cloud-based communications solutions. Insight therefore advised that these locations were best served by retaining their PBX system. Insight’s technical teams configured the solution that combined a service developed by a leading international provider of Unified-Communications-as-a-Service (UCaaS) with PBX maintenance services from a local company based in Kilmarnock. Insight developed a simplified billing approach for the solution. The council benefits from the fixed cost of SIP telephony, including a package of call minutes, and receives a single invoice for its telephony and communication services rather than having to process multiple invoices from several suppliers.

North Ayrshire wanted a Session Initiation Protocol (SIP) trunking service to enable voice over IP (VoIP) connectivity with the public switched telephone network (PSTN). This would provide the council with the flexibility it needed to allow staff to work from anywhere, communicating via laptops and headsets, and also provide an easy forward step to cloud-based telephony in the future. The project was put out to a mini competitive tender in 2019 through the Scottish government’s dynamic purchasing system (DPS). The council evaluated submissions from three providers, from which Insight was selected, scoring highly on the ratio of cost to quality.

Following a successful tender process, Insight was selected to upgrade the unified communications platform for North Ayrshire Council.

North Ayrshire Council wanted to upgrade to a Unified Communications platform to improveits resilience and flexibility. Its existing system was made up of multiple disparate PBXsystems. As these weren’t part of a centralised corporate support package, costs were high,and the aging hardware was at risk of no longer being supported by the supplier.As part of a broader modernisation programme, the council was also looking to improve communications for both staff and citizens. Simplifying the system for citizens was a priority.

England has 42 Integrated Care Systems under the Health and Care Act (2022)
Ultimately, all the technology used across health and social care has a single aim – to help people live longer, healthier lives. Technology will never replace the human touch or the empathy that a nurse has when holding your hand as they tell you about your scan results or the outcome of your child’s surgery. What it can do, is support hard-working individuals across the service – from people on the frontline to those who provide back-office support including admin, security, procurement and more. There is a workforce crisis across the whole of the health service that technology can play a part in helping to address by alleviating the workload and supplementing staff…never replacing them. The combined impact of clever individual technologies and collaborative, infrastructure-level environments, returns a greater opportunity to scale and accelerate innovation to help ICSs succeed. Technology can help to improve health outcomes while alleviating some of the pressure on the workforce. Strategic partnerships with innovative, dedicated providers like CISCO and Insight are part of the solution.
The combinatorial effect
Procurement that occurs at a ‘place or system level’ rather than by the hospital, the local authority, or some other body, will ultimately have a more meaningful impact on healthcare outcomes because it ties together the people and local communities who use the services, underpinning the integrated models of care that will enable patients to be seen in, or close to, the place where they live. According to The King’s Fund, an independent charitable organisation that works to improve health and care in England, ‘place-based’ partnerships are the foundation of effective Integrated Care Systems. They focus on the relationships between systems, places and neighbourhoods. As health care consumers, i.e., people, we relate better to this more holistic yet local focus, rather than having to navigate disparate bodies with responsibility for different elements of the health service.
Collaborative procurement for greater impact
An important factor in all of this is the technical maturity of the health and care services, and of the provider as well. A mature technology and systems provider, like Insight, is ideally placed to help ICSs develop and deliver the right technical solutions and also help in the procurement process. Combined procurement – between the different parts of the ICS – will help avoid the risk of duplicating or conflicting systems and ensure that best value is delivered. Take, for example, the major initiative to roll out electronic patient records, or EPR. Purchasing the software is one thing, but if the supporting technology like the Wi-Fi on the ward where the patient’s data is collected isn’t working, you’re not going to get any benefit. In fact, you’re going to create more work for already pressured frontline staff. An integrated approach to technology provides the full picture and helps the ICS get the true benefit from its technical procurement.
While there is limitless opportunity for technology to support the ICSs to improve health outcomes and reduce inequalities, ICBs will need to ensure they have the technical integration in place that brings disparate systems together to support new ways of working at system and place. To be successful with digital transformation, ICSs need an integrated cross-architecture infrastructure that bridges the partner organisations. It’s about more than the data centre or networking, more than security or telephony. It’s how those elements work together across a region. I would really emphasise the need for ICSs to look at their technical infrastructure in the same way as they organise their care systems and services – avoiding duplication, streamlining processes, ensuring consistency, security, and compliance, etcetera.
Integrated care calls for integrated technology
We are seeing an acceleration in innovation, particularly when we look at solutions for treating patients in their own homes. These can be quite creative solutions, some even using consumer-based technology like Alexa and IoT. They can help reduce the backlog of care by making it safer to send patients home with a ‘digital discharge’ package or enabling patients to avoid hospitalisation altogether by managing their care at home or on a virtual ward. Even something as simple as technology that shows a spike in electricity usage when a kettle is switched on indicates that a person is up and about in their own home. In the hospital environment, Bluetooth tracking that shows where a patient is, can be invaluable in helping staff to keep the waits in A&E to a minimum, or helping to keep patients safe, such as those with dementia that might be at risk of wandering. It can also be used to track certain medical devices that always seem to go walkabout. Most nurses would recognise that on a typical 12-hour shift they can spend an hour walking around looking for something like a bladder scanner.
As a CIO and digital lead within the NHS for many years before joining CISCO, I know how accomplished the NHS is when it comes to innovation. Insight has been at the heart of some key innovations by providing equipment and capabilities in projects like a secure website for COVID-19 aftercare and enhanced learning using immersive technology.
The maturity of solution providers aids combined procurement
Innovation drives advances in care
Integrated Care Systems are partnerships of local organisations that come together to plan and deliver better, more joined up care for patients and improvements in population health. Forty-two ICSs were established in England when the Health and Care Act (2022) was passed. ICSs include Integrated Care Partnerships (ICP), Integrated Care Boards (ICB), local authorities and place-based partnerships that can include NHS, local councils, community and voluntary organisations, residents, service users, careers and others. As you can imagine, bringing these different organisations together under a single design framework is a major undertaking. Likewise, getting the technical infrastructure to support the ICS design framework has been a real challenge for CIOs & CTOs across the board.

One thing I learnt during my time within the NHS is that change is inevitable. Changes of scale, like the Health Care Act (2022) can sometimes be met with resistance, but to my mind, this act is an intelligent and well thought through piece of legislation. Integrated care is the only way we are going to address the challenges of the healthcare system going forward, and technology plays an integral part. Before a brief explanation of what Integrated Care Systems (ICSs) are, it’s important to understand that the Act is the result of a long, democratic consultation and is not a surprise to the health service. Most ICSs already have produced – or are in the process of producing their digital plan – to deliver the ICSs’ purpose, which is to bring partner organisations together to:
Improve outcomes in population health and healthcare Tackle inequalities in outcomes, experience and access Enhance productivity and value for money Help the NHS support broader social and economic development.
