All three plans have the same list of covered health care services and prescription drugs. Though the level of coverage varies on each plan, there is no service or prescription drug that is covered on one plan but not another.

Overview of Skanska’s PPO plans:
Overview of Skanska HDHP plan:
If you enroll in a PPO plan, you’ll pay higher premiums to have the plan. But you’ll have lower costs when you use the plan. On our PPO plans, most services require only a copay at the time the care is received. Some services may require you satisfy a minimal deductible, which means you’ll pay the full cost of the service until you’ve met the deductible. After you pay the applicable copay or deductible, all in-network services and prescription drugs are covered 100% by Anthem. The primary advantage of PPO plans is the low out of pocket cost when you use the plan. But you should also assess how this low cost of care balances against the high contributions that are deducted from your paycheck to have the plan.
If you enroll in the HDHP plan, you’ll pay the lowest premiums to have the plan. But you’ll have a highest costs when you use the plan. On the HDHP plan, all services are subject to the deductible. This means you’ll be responsible for paying the full cost of all services or prescriptions drugs out of pocket until you’ve met the deductible. After you meet the deductible: - All in-network services on the HDHP plan are covered 80% by Anthem. This means you’ll receive a bill for the remaining 20% of the cost. - All prescription drugs are covered 100% by Anthem after you pay the applicable copay. As you pay your deductible, copay and 20% coinsurance amounts, Anthem tracks these out-of-pocket costs. If at any point your costs meet the out-of-pocket maximum on the plan, Anthem will pay 100% of the costs of your care and prescriptions through the end of the year. One advantage of the HDHP plan is the low cost to have the plan. But you should assess how these low contributions from your paycheck balance against the higher costs you’re responsible at time of care. An additional advantage of the HDHP plan is your ability to contribute to a Health Savings Account (HSA). HSAs offer “triple tax savings” opportunities. Skanska also contributes to your HSA to help buffer the high cost of the deductible. Learn more about HSAs by reviewing page 12 of the 2024 USB/CDUS/USA Inc. Skanska benefits programs and prescription details
When it comes to work-life support, Skanska wants to be part of your solution. Plan enhancements, benefit features and prescription details are outlined below. As a reminder, you must be enrolled in one of Skanska’s medical plans to utilize the following benefits.
Travel & lodging reimbursement Provides reimbursement for mileage and lodging expenses in the event you or a covered dependent need to receive care, but there are no in network providers located within a 100-mile radius of your home address. Reimbursements are based upon IRS mileage and lodging rates, not to exceed $3,000 per eligible occurrence.
Expanded family planning Anthem’s lifetime benefit maximum for fertility services is $30,000 and this amount includes financial assistance for adoptions, surrogacy costs and fertility preservation. Our family planning benefits also include access to WINFertility, a dedicated care management team, to help you maximize these benefits based on your unique needs. Anthem’s fitness reimbursement You and your eligible dependents (18+) enrolled in any of the medical plans are eligible to receive up to $400 per year ($200 per 6-month period) in reimbursement for fitnessrelated membership expenses. Reimbursements are available for both “brick-and-mortar” (YMCA, LA Fitness, Soul Cycle, etc.) and online/virtual memberships (Peloton, Daily Yoga, etc.). Members must provide proof of 50 visits per 6-month period.
Virtual primary care Through Anthem’s SydneySM Health mobile app, you and your covered dependents can receive comprehensive primary care coordinated by a care team, preventive care through wellness check-ins and lab screenings, a personalized care plan and follow-ups, unlimited access to care including prescription refills and referrals, and 24/7 on-demand urgent care support.
Total Health Select Connects you to Anthem’s dedicated advocates, experts in specialties like behavioral health, cancer, NICU, transplants, and more. You have access to a team of health coaches, nurses, and family support specialists providing you customized healthcare solutions. To access, use the SydneySM health mobile app.

Virtual in-network visits When virtually visiting an in-network provider, you will be billed at the same cost as an in-person visit. You’ll receive the same care from familiar providers without leaving the comfort of your own home.
Mail order prescriptions Available for maintenance drugs, those prescribed for an extended period of time to treat chronic conditions (e.g. high blood pressure). By signing up for mail order, you’ll receive a 3-month supply of your prescription at your home address, and only pay the cost of a 2-month supply. To initiate, call 1-833-226-1114 or register at anthem.com.
Prescription cost relief Cost relief allows you to fill certain specialty medications at no cost to you when filled through CarelonRx Specialty Pharmacy. If you or a family member are currently prescribed a specialty drug that is eligible for cost relief, you will receive a letter directly from Anthem with more information and instructions to enroll.
Prescription step therapy Skanska’s prescription plan includes step therapy, which requires use of generic or lower cost equivalent drugs when they are available.
Skanska offers three medical plans through Anthem Blue Cross Blue Shield (Anthem) – The PPO1 Plan, the PPO2 plan and the High Deductible Health Plan. Below are three scenarios that were developed to show how the same medical claims will process differently and result in varying costs and out of pocket responsibilities under each medical plan. The scenarios include a "minimal" care example, a "moderate" care example, and a "significant" or "catastrophic" care example. Review the disclaimer at the bottom of the page for additional information.
In this example, you use your Skanska health insurance to receive a minimal amount of care throughout the year. You complete your annual preventive care and bloodwork, and you have two non-routine visits with your PCP and a specialist. You fill a few short-term prescriptions and you receive an x-ray during an urgent care visit one evening.
"Minimal" Claims Utilizer
In this example, you use your Skanska health insurance to receive a moderate amount of care throughout the year. You complete your annual preventive care and routine bloodwork. You have a number of additional office visits with your PCP and regularly fill a maintenance medication prescription drug. You see a chiropractor a number of times during the year due to chronic pain. You ultimately see a specialist doctor and receive an MRI. Finally, you have a same-day outpatient procedure at a surgical center. Afterward, you fill multiple short-term prescriptions and have multiple follow-up visits with a specialist.
"Moderate" Claims Utilizer
In this example, you use your Skanska health insurance to receive a significant amount of care throughout the year. You complete your annual preventive care and routine bloodwork. You have a number of diagnosed chronic health conditions that require regular specialist visits and higher-cost prescription drugs. You have an urgent care visit when experiencing concerning symptoms. You ultimately experience a catastrophic health event during the plan year, resulting in a 3-month inpatient hospital stay. After your release from the hospital, you require regular specialist visits, follow-up bloodwork and MRIs, physical and occupational therapy.

You each have an urgent care visit on separate occasions. The urgent care visits each include an x-ray and short-term prescription antibiotic fills. Finally, you each see a specialist doctor and a chiropractor on a minimal number of occasions during the year.
In this example, you and your spouse/partner use your Skanska health insurance to receive a moderate amount of care throughout the year. You both complete your annual preventive care and routine bloodwork. Both of you receive an irregular result from one of your preventive care screenings, requiring multiple followup visits with your PCPs and specialists, as well as additional labwork and scans. Your spouse ultimately requires a same-day outpatient procedure as a result. You each visit a specialist regularly for chronic condition management. This care includes visits with multiple specialists, and you both take a maintenance medication prescription drug. Finally, one of you has an urgent care visit one evening, resulting in stitches. The other experiences a fall resulting in the need for an MRI, specialist visits, and ultimately a same-day outpatient procedure at a surgical center.
In this scenario, you and your spouse/partner use your Skanska health insurance to receive a significant amount of care throughout the year. You both complete your annual preventive care and routine bloodwork. You both multiple have chronic health conditions that require various maintenance medications and specialty prescription drugs. Your health conditions require regular visits to your PCPs and various specialists. One of you suffers a fall, resulting in an urgent care visit, specialist office visits, physical therapy and multiple MRIs. You ultimately receive an outpatient procedure at a surgical center but develop a severe infection while recovering. This results in an inpatient hospital stay. Finally, both you and your spouse experience concerning symptoms later in the year and are admitted to the hospital, resulting in lengthy hospital stays. The inpatient stays include a series of imaging tests, medications and high-cost treatment. You both recover and are ultimately discharged, and you return to visiting your doctors according to your regular care schedules.
In this example, you and one child use your Skanska health insurance to receive a minimal amount of care throughout the year. You both complete your annual preventive care and routine bloodwork. During the year, your child has a few sick visits with your pediatrician and is prescribed an antibiotic each time. You receive an irregular preventive care result and have a followup with your PCP and a specialist. Your followup bloodwork results indicate no further concern. You and your child each have an urgent care visit during the year, resulting in x-rays and antibiotics.
In this example, you and one child use your Skanska health insurance to receive a moderate amount of care throughout the year. You both complete your annual preventive care and routine bloodwork. You each have a chronic health condition requiring various primary care and specialist visits throughout the year, as well as generic maintenance medications to help manage your conditions. Your child also has a few sick visits during the year with your pediatrician. You and your child each have an unexpected urgent care visit during the year and receive treatment while in the urgent care. You suffer from chronic pain and regularly see a chiropractor. After experiencing a fall, you require specialist visits, a prescription, an MRI and ultimately an outpatient procedure at a surgical center.
In this example, you and one child use your Skanska health insurance to receive a signigicant amount of care throughout the year. You both complete your annual preventive care and routine bloodwork. You each have a chronic health condition requiring various primary care and specialist visits throughout the year, as well as maintenance medications. During the year, you each begin visit specialists on a monthly basis to focus on mental wellbeing and lifestyle changes. One of you visits an urgent care when needing an xray. You follow up with a specialist and receive an MRI, and ultimately require a surgical procedure. Your recovery includes physical therapy, prescription medications and follow-up specialist visits. Finally, one of you experiences a severe accident during the year resulting in a lengthy inpatient hospitalization, including imaging tests, medications and high-cost treatment. Your recovery additionally includes months of physical and occupational therapy visits, as well as high-cost medications.
In this example, you and your two children use your Skanska health insurance to receive a minimal amount of care throughout the year. You all complete your annual preventive care and routine bloodwork. You and one child require followup care as a result of irregular preventive screening results. In this example, you and your two children use your Skanska health insurance to receive a significant amount of care throughout the year. You all complete your annual preventive care and routine bloodwork. You require followup care as a result of irregular preventive screening results, resulting in multiple specialist visits, a prescription maintenance medication and a routine outpatient procedure. One of your children has a chronic health condition, and they visit a specialist on a monthly basis and you regularly fill a high-cost specialty drug for their care management. Your family has a number of urgent care visits during the year for varying needs, including an x-ray and a health scare for your child with chronic care needs. You are prescribed multiple maintenance medications and are referred to a specialist, who you see every few months. Have a few sick visits, as well as urgent care visits and x-rays during the year, for various mild accidents and symptoms. Following one of the visits, one of your children requires follow-up care with a specialist, as well as an outpatient procedure and physical therapy.
In this example, you, your spouse/partner and your three children use your Skanska health insurance to receive a significant amount of care throughout the year. These inpatient hospitalization are lengthy and the recovery is slow and requires much care coordination, including inpatient rehabilitation, prescriptions and regular follow-up visits.

Available only to those enrolled in the HDHP + HSA medical plan
A health savings account (HSA) is a tax-free savings account that belongs to you. HDHP + HSA medical plan enrollees are able to use an HSA to pay for insurance deductibles and out-of-pocket healthcare related expenses. To be eligible for the HSA, you must: • Be enrolled in the HDHP + HSA medical plan • Not be enrolled in Medicare Parts A or B • Not be claimed as a dependent on someone else’s tax return • Have no other insurance If you enroll in the HDHP + HSA medical plan,Skanska will automatically contribute the belowamount towards your HSA. You also have the optionto contribute the difference between Skanska’scontribution and the IRS maximum.
Once enrolled in the HDHP + HSA medical plan, you will receive your HSA PayFlex debit card in the mail, along with a welcome letter instructing you how to get started. Most purchases can be made and substantiated using the PayFlex-issued debit card. You may also visit payflex.com to file your claim online.
How do I access my funds?
The money in the account can be used for qualified medical expenses (as defined by the IRS) or can be left to grow with tax free interest from year to year. The money will be kept in an FDIC insured bank account or when the balance is high enough that it can be invested in mutual funds. Additionally, you can make a withdrawal for non-medical expenses when you reach the age of 65. Prior to age 65, any non-qualified purchases will be taxed and subject to a 20% penalty.
What expenses can it pay for?
Most purchases can be made and substantiated using the PayFlex-issued debit card. You may also visit payflex.com to file your claim online. If you are mailing a paper claim, please make sure the address on the form is: PayFlex Systems USA Inc., P.O. Box 981158, El Paso, Texas 79998.

Individuals over age 55 may make an additional ‘catch-up’ contribution of $1,000 per year. Those age 64+ are potentially subject to unique restrictions. Please contact 866-SKANSKA for more information.
How do funds accumulate?
You can link one or more of your bank accounts to and from your HSA to easily transfer funds to and from your HSA. Once logged in, go to Account Settings > Bank accounts > Link Bank Account to my HSA. Select the account, and enter your account number and routing number. Authorize PayFlex to link your account and click Save and Continue.
Do unused funds roll over?
All unused HSA funds or your entire HSA balance will roll over from year to year, even if you don’t spend them. Should you leave employment or change your enrollment to a non-HDHP, you are still able to draw funds from the account.

In this example, an employee uses their Skanska health insurance for a significant amount of additional care throughout the year. • You complete their annual preventive care and routine bloodwork. • You have Type 2 Diabetes and fill a brand name prescription each month, totaling 12 brand prescription fills throughout the year. • You visit an in-network mental health provider once per month, totaling 12 times during the year. • You visit their in-network primary care physician (PCP) every six months to check-in on your Diabetes, so they have two PCP office visits in addition to your routine preventive care visit. • After the second PCP visit, your PCP prescribes a new generic prescription and refers them for bloodwork. You visit an in-network lab to complete the bloodwork, and fills your new prescription six times before the end of the year. • You unfortunately experience a severe stroke and are admitted to an in-network hospital.
