The primary reason for an image being out of focus is usually caused by the camera shaking while shooting. Be sure to set the camera on a solid tripod.
Blurring can also occur due to a faulty autofocus system within the camera itself. The camera will have difficulty capturing the focus on the low-contrast areas of the flat images. Hone in on a rich in contrast area of work or edge and then take the picture to avoid this problem.

Blurring
Uneven lighting occurs when shooting with light coming in from an angle. Similarly, setting the photographed image opposite the window will cause ghosting. Block-out curtains should be used in the studio while shooting.
Uneven lighting
Reflections and glare can spoil any photograph. Light reflectors cause directional light to fall on the glass or other reflecting surfaces. One way to avoid this is using a lens filter.
Reflections
If colour discolouration on the reproduced work has occurred, check the colour temperature of the current lighting. Colour fidelity can be corrected in print by using the white reference from the colorchecker.
In the morning and afternoon, the sunlight "warms", while the blue and non-cloudy sky causes the "cold" colors.
Accurate colour representation in printing can only be achieved by using
a color checker, after which modifications can be done via professional colour balancing software such as Photoshop.
Colour Correction
Even the best automatic metering system can fail sometimes.
Lack of detail in bright portions of the image, or conversely, too much contrast can be an issue with incorrect
exposure levels.
If darker portions of the image lack detail, and overall, the image has low contrast, use a colorchecker and grayscale strip to measure values
and correct accordingly.

The higher the quality, the better. The best format for images to be saved in, ready for further modifications is RAW.
Low resolution, poor quality
There are various types of distortion, mostly caused by the camera lens.
These include:
Lens quality plays a huge part in avoiding one of the many types of distortion caused while shooting two-dimensional pieces. Invest in as high-quality lens as you can afford.
Vignetting - occurs when the corners of an image appear darkened as if a shadow were surrounding the photo.
Chromatic aberration (sometimes known as fringing) - produces color fringing around the edges of high contrast images.
Barrel distortion - images appear to have been wrapped around a barrel and thus the center of the image appears larger than the edges.
Lens flaring - creates a pattern of light straying across the camera lens, caused by a very strong
light source
